Iceman may have suffered from Lyme disease

The 5,300-year-old ice mummy dubbed Otzi, discovered in the Eastern Alps about 20 years ago, appears to have had the oldest known case of Lyme disease, new genetic analysis has revealed.

As part of work on the Iceman's genome ? his complete genetic blueprint ? scientists found genetic material from the bacterium responsible for the disease, which is spread by ticks and causes a rash and flulike symptoms and can lead to joint, heart and nervous system problems.

The new analysis also indicates the Iceman was lactose intolerant, predisposed to cardiovascular disease, and most likely had brown eyes and blood type O.

To sequence the Iceman's genome, researchers took a sample from his hip bone. In it, they looked for not only human DNA ? the chemical code that makes up genes ? but also for that of other organisms. While they found evidence of other microbes, the Lyme disease bacterium, called Borrelia burgdorferi, was the only one known to cause disease, said Albert Zink, a study researcher and head of the European Institute for Mummies and the Iceman at the European Academy of Bozen/Bolzano (EURAC) in Italy.??

"Our data point to the earliest documented case of a B. burg?dorferi infection in mankind. To our knowledge, no other case report about borreliosis (Lyme disease) is available for ancient or historic specimens," Zink and colleagues write in an article published on Tuesday in the journal Nature Communications.

Discovering evidence of Borrelia is an "intriguing investigative lead," said Dr. Steven Schutzer, an immunologist at the University of Medicine and Dentistry of New Jersey-New Jersey Medical School.

Schutzer is a lead investigator on a National Institutes of Health-funded project that has sequenced at least 17 strains of the modern bacterium, and has published 13 of those so far.

The discovery of the traces of Borrelia within the sample taken from the Iceman still needs to be confirmed, he said. "Now we know what we want to look for, now that we know there is a possibility of that being here, we can do a very targeted approach that looks for Borrelia," Schutzer said.

Lyme disease is transmitted by ticks in North America and Eurasia. It was first found in the United States in Connecticut in the mid-1970s; a similar disorder had been identified in Europe earlier in the 20th century.?

Schutzer said he is discussing follow-up studies with Zink.

Previous work had examined genetic material within the Iceman's mitochondria ? the energy-producing centers in cells. His mitochondrial DNA, which is inherited through the maternal line, did not reveal any living relatives.

In this new project, researchers decoded the DNA found within the nuclei of the Iceman's cells, which is inherited from both parents. They found the Iceman belonged to a lineage that is now rare, but still present in some places.

"This means his ancestors came from Europe originally from the East and spread over most or part of Europe," Zink said. "This original population was somehow replaced by other populations, but they remained quite stable in remote areas like Sardinia and Corsica."

The analysis also indicates the Iceman was lactose intolerant. This isn't surprising, according to Zink. At the time the Iceman lived and died, people were beginning to settle down and become farmers, and the ability for adults to digest milk became an advantage.

The team also found he had a genetic predisposition for cardiovascular disease, supporting earlier scans showing the buildup of deposits within his arteries. In spite of the health problems from which the roughly 45-year-old Iceman suffered, he appears to have died a violent death. Researchers believe a flint arrowhead, shot into his left shoulder most likely killed him.

Going Beyond the Tale of a Boy and His Horse

Well in advance of the 2014 centennial of the beginning of ?the war to end all wars,? the First World War is suddenly everywhere in our lives. Stephen Spielberg?s War Horse opened on 2,376 movie screens and has collected six Oscar nominations, while the hugely successful play it?s based on is still packing in the crowds in New York and a second production is being readied to tour the country.

In addition, the must-watch TV soap opera of the last two months, Downton Abbey, has just concluded its season on an unexpected kiss.? In seven episodes, its upstairs-downstairs world of forbidden love and dynastic troubles took American viewers from mid-war, 1916, beyond the Armistice, with the venerable Abbey itself turned into a convalescent hospital for wounded troops. Other dramas about the 1914-1918 war are on the way, among them an HBO-BBC miniseries based on Ford Madox Ford?s Parade?s End quartet of novels, and a TV adaptation of Sebastian Faulks?s novel Birdsong from an NBC-backed production company.

In truth, there?s nothing new in this.? Filmmakers and novelists have long been fascinated by the way the optimistic, sunlit, pre-1914 Europe of emperors in plumed helmets and hussars on parade so quickly turned into a mass slaughterhouse on an unprecedented scale. And there are good reasons to look at the First World War carefully and closely.

After all, it was responsible for the deaths of some nine million soldiers and an even larger number of civilians.? It helped ignite the Armenian genocide and the Russian Revolution, left large swaths of Europe in smoldering ruins, and remade the world for the worse in almost every conceivable way -- above all, by laying the groundwork for a second and even more deadly, even more global war.

There are good reasons as well for us to be particularly haunted by what happened in those war years to the country that figures in all four of these film and TV productions: Britain. In 1914, that nation was at the apex of glory, the unquestioned global superpower, ruling over the largest empire the world had ever seen. Four and a half years later its national debt had increased tenfold, more than 720,000 British soldiers were dead, and hundreds of thousands more seriously wounded, many of them missing arms, legs, eyes, genitals.

The toll fell particularly heavily on the educated classes that supplied the young lieutenants and captains who led their troops out of the trenches and into murderous machine-gun fire. To give but a single stunning example, of the men who graduated from Oxford in 1913, 31% were killed.

?Swept Away in a Red Blast of Hate?

Yet curiously, for all the spectacle of boy and horse, thundering cavalry charges, muddy trenches, and wartime love and loss, the makers of War Horse, Downton Abbey and -- I have no doubt -- the similar productions we?ll soon be watching largely skip over the greatest moral drama of those years of conflict, one that continues to echo in our own time of costly and needless wars. They do so by leaving out part of the cast of characters of that moment.? The First World War was not just a battle between rival armies, but also a powerful, if one-sided, battle between those who assumed the war was a noble crusade and those who thought it absolute madness.

The war?s opponents went to jail in many countries.? There were more than 500 conscientious objectors imprisoned in the United States in those years, for example, plus others jailed for speaking out against joining the conflict. Eugene V. Debs had known prison from his time as a railway union leader, but he spent far longer behind bars -- more than two years -- for urging American men to resist the draft. Convicted of sedition, he was still in his cell at the federal penitentiary in Atlanta in November 1920 when, long after the war ended, he received nearly a million votes as the Socialist candidate for President.

One American protest against the war turned to tragedy when, in 1917, Oklahoma police arrested nearly 500 draft resisters -- white, black, and Native American -- taking part in what they called the Green Corn Rebellion against ?a rich man?s war, poor man?s fight.? Three were killed and many injured.

War resisters were also thrown in jail in Germany and Russia. But the country with the largest and best organized antiwar movement -- and here?s where the creators of those film and TV costume dramas so beloved by Anglophile American audiences miss a crucial opportunity -- was Britain.

The main reason opposition to the war proved relatively strong there was simple enough: in 1914, the island nation had not been attacked. German invaders marched into France and Belgium, but Germany hoped Britain would stay out of the war. And so did some Britons. When their country joined the fighting on the grounds that Germany had violated Belgian neutrality, a vocal minority continued to insist that jumping into a quarrel among other countries was a disastrous mistake.

Keir Hardie was a prominent early war opponent.? A trade union leader and Member of Parliament, he had, by the age of 21, already spent half his life as a coal miner and he never went to school.? Nonetheless, he became one of the great orators of the age, mesmerizing crowds with his eloquence, his piercing, heavy-browed eyes, and a striking red beard. Crushed with despair that millions of Europe?s working men were slaughtering one another rather than making common cause in fighting for their rights, his beard white, he died in 1915, still in his 50s.

Among those who bravely challenged the war fever, whose rallies were often violently broken up by the police or patriotic mobs, was well-known radical feminist Charlotte Despard. Her younger brother, amazingly, was Field Marshal Sir John French, commander-in-chief of the Western Front for the first year and a half of the war. A similarly riven family was the famous Pankhurst clan of suffragettes: Sylvia Pankhurst became an outspoken opponent of the conflict, while her sister Christabel was from the beginning a fervent drum-beater for the war effort.? They not only stopped speaking to each other, but published rival newspapers that regularly attacked the other?s work.

Britain?s leading investigative journalist, Edmund Dene Morel, and its most famous philosopher, Bertrand Russell, were both passionate war critics. ?This war is trivial, for all its vastness,? Russell wrote. ?No great principle is at stake, no great human purpose is involved on either side.? He was appalled to see his fellow citizens ?swept away in a red blast of hate.?

He wrote with remarkable candor about how difficult it was to go against the current of the national war fever ?when the whole nation is in a state of violent collective excitement. As much effort was required to avoid sharing this excitement as would have been needed to stand out against the extreme of hunger or sexual passion, and there was the same feeling of going against instinct.?

Both Russell and Morel spent six months in prison for their beliefs. Morel served his term at hard labor, carrying 100-pound slabs of jute to the prison workshop while subsisting on a bare-bones diet during a frigid winter when prison furnaces were last in line for the nation?s scarce supply of coal.

Women like Violet Tillard went to jail as well.? She worked for an antiwar newspaper banned in 1918 and was imprisoned for refusing to reveal the location of its clandestine printing press. And among the unsung heroines of that antiwar moment was Emily Hobhouse, who secretly traveled through neutral Switzerland to Berlin, met the German foreign minister, talked over possible peace terms, and then returned to England to try to do the same with the British government. Its officials dismissed her as a lone-wolf eccentric, but in a conflict that killed some 20 million people, she was the sole human being who journeyed from one side to the other and back again in search of peace.

Why We Know More About War Than Peace

By the war?s end, more than 20,000 British men had defied the draft and, as a matter of principle, many also refused the alternative service prescribed for conscientious objectors, like ambulance driving at the front or working in a war industry. More than 6,000 of them were put behind bars -- up to that moment the largest number of people ever imprisoned for political reasons in a western democracy.

There was nothing easy about any of this.? Draft refusers were mocked and jeered (mobs threw rotten eggs at them when given the chance), jailed under harsh conditions, and lost the right to vote for five years. But with war?s end, in a devastated country mourning its losses and wondering what could possibly justify that four-year slaughter, many people came to feel differently about the resisters. More than half a dozen were eventually elected to the House of Commons and the journalist Morel became the Labour Party?s chief Parliamentary spokesperson on foreign affairs. Thirty years after the Armistice, a trade unionist named Arthur Creech Jones, who had spent two and a half years in prison as a war resister, was appointed to the British cabinet.

The bravery of such men and women in speaking their minds on one of the great questions of the age cost them dearly: in public scorn, prison terms, divided families, lost friends and jobs. And yet they are largely forgotten today at a moment when resistance to pointless wars should be celebrated. ?Instead we almost always tend to celebrate those who fight wars -- win or lose -- rather than those who oppose them.

It?s not just the films and TV shows we watch, but the monuments and museums we build. No wonder, as General Omar Bradley once said, that we ?know more about war than we know about peace.? We tend to think of wars as occasions for heroism, and in a narrow, simple sense they can be. But a larger heroism, sorely lacking in Washington this last decade, lies in daring to think through whether a war is worth fighting at all. In looking for lessons in wars past, there?s a much deeper story to be told than that of a boy and his horse.

Metal nanoparticles shine with customizable color

ScienceDaily (Feb. 23, 2012) ? Engineers at Harvard have demonstrated a new kind of tunable color filter that uses optical nanoantennas to obtain precise control of color output.

Whereas a conventional color filter can only produce one fixed color, a single active filter under exposure to different types of light can produce a range of colors.

The advance has the potential for application in televisions and biological imaging, and could even be used to create invisible security tags to mark currency. The findings appear in the February issue of Nano Letters.

Kenneth Crozier, Associate Professor of Electrical Engineering at the Harvard School of Engineering and Applied Sciences (SEAS), and colleagues have engineered the size and shape of metal nanoparticles so that the color they appear strongly depends on the polarization of the light illuminating them. The nanoparticles can be regarded as antennas -- similar to antennas used for wireless communications -- but much smaller in scale and operating at visible frequencies.

"With the advances in nanotechnology, we can precisely control the shape of the optical nanoantennas, so we can tune them to react differently with light of different colors and different polarizations," said co-author Tal Ellenbogen, a postdoctoral fellow at SEAS. "By doing so, we designed a new sort of controllable color filter."

Conventional RGB filters used to create color in today's televisions and monitors have one fixed output color (red, green, or blue) and create a broader palette of hues through blending. By contrast, each pixel of the nanoantenna-based filters is dynamic and able to produce different colors when the polarization is changed.

The researchers dubbed these filters "chromatic plasmonic polarizers" as they can create a pixel with a uniform color or complex patterns with colors varying as a function of position.

To demonstrate the technology's capabilities, the acronym LSP (short for localized surface plasmon) was created. With unpolarized light or with light which is polarized at 45 degrees, the letters are invisible (gray on gray). In polarized light at 90 degrees, the letters appear vibrant yellow with a blue background, and at 0 degrees the color scheme is reversed. By rotating the polarization of the incident light, the letters then change color, moving from yellow to blue.

"What is somewhat unusual about this work is that we have a color filter with a response that depends on polarization," says Crozier.

The researchers envision several kinds of applications: using the color functionality to present different colors in a display or camera, showing polarization effects in tissue for biomedical imaging, and integrating the technology into labels or paper to generate security tags that could mark money and other objects.

Seeing the color effects from current fabricated samples requires magnification, but large-scale nanoprinting techniques could be used to generate samples big enough to be seen with the naked eye. To build a television, for example, using the nanoantennas would require a great deal of advanced engineering, but Crozier and Ellenbogen say it is absolutely feasible.

Crozier credits the latest advance, in part, to taking a biological approach to the problem of color generation. Ellenbogen, who is, ironically, colorblind, had previously studied computational models of the visual cortex and brought such knowledge to the lab.

"The chromatic plasmonic polarizers combine two structures, each with a different spectral response, and the human eye can see the mixing of these two spectral responses as color," said Crozier.

"We would normally ask what is the response in terms of the spectrum, rather than what is the response in terms of the eye," added Ellenbogen.

Researchers discover how vitamin D inhibits inflammation

Researchers at National Jewish Health have discovered specific molecular and signaling events by which vitamin D inhibits inflammation. In their experiments, they showed that low levels of Vitamin D, comparable to levels found in millions of people, failed to inhibit the inflammatory cascade, while levels considered adequate did inhibit inflammatory signaling. They reported their results in the March 1, 2011, issue of The Journal of Immunology.

"This study goes beyond previous associations of vitamin D with various health outcomes. It outlines a clear chain of cellular events, from the binding of DNA, through a specific signaling pathway, to the reduction of proteins known to trigger inflammation," said lead author Elena Goleva, assistant professor of pediatrics at National Jewish Health. "Patients with chronic inflammatory diseases, such as asthma, arthritis and prostate cancer, who are vitamin D deficient, may benefit from vitamin D supplementation to get their serum vitamin D levels above 30 nanograms/milliliter."

Current national guidelines suggest that people should maintain a minimum blood serum level of 20 ng/ml, although there is much scientific debate about optimum levels. Vitamin D has long been known to contribute to bone health by promoting the absorption of calcium. In recent years, much attention has been paid to its possible immune and inflammatory benefits. Low vitamin D levels have been associated with several diseases including asthma, cancer, diabetes, and arthritis.

In the current study researchers examined the specific mechanisms by which vitamin D might act on immune and inflammatory pathways. They incubated human white blood cells with varying levels of vitamin D, then exposed them to lipopolysaccharide (LPS), a molecule associated with bacterial cell walls that is known to promote intense inflammatory responses.

Cells incubated with no vitamin D and in solution containing 15 ng/ml of vitamin D produced high levels of cytokines IL-6 and TNF-?, major actors in the inflammatory response. Cells incubated in 30 ng/ml vitamin D and above showed significantly reduced response to the LPS. The highest levels of inflammatory inhibition occurred at 50 ng/ml.

Through a complex series of experiments, the researchers identified a new location where the vitamin-D receptor appears to bind directly to DNA and activate a gene known as MKP-1. MKP-1 interferes with the inflammatory cascade triggered by LPS, which includes a molecule known as p38, and results in higher levels of IL-6 and TNF-?.

"This newly identified DNA-binding site for the vitamin-D receptor, and the specific pathways inhibited by higher levels of vitamin D provide a plausible mechanism for many of the benefits that have been associated with vitamin D," said Dr. Goleva. 'The fact that we showed a dose-dependent and varying response to levels commonly found in humans also adds weight to the argument for vitamin D's role in immune and inflammatory conditions."

As a freelance sports writer, I?ve been lucky enough to enjoy a game at every MLB, NFL, NBA and NHL venue, plus hundreds of college and minor league sites. And while I?m no economist or prognosticator, I can envision MLB leaving St. Petersburg, Fla. come the 2028 season, if not sooner.

But where? There are three sets of questions to ask when discussing MLB relocation.

1. Factor: Population Size.

How big is the metropolitan area? In MLB, size matters. Of the 30 current MLB teams, only seven are located in markets with less than 3 million people in their respective greater metropolitan areas, according to the 2010 U.S. Census (Cleveland, St. Louis, Tampa Bay, Pittsburgh, Cincinnati, Kansas City and Milwaukee). Small markets that host an NHL and NBA team might be able to get away with it because they play only half as many home games than the 81 games baseball teams do, plus MLB facilities are two to three times bigger, so more fannies can be in the seats.

In the U.S. and Canada, there?s only one market in the U.S. and Canada without an MLB team and a metropolitan population greater than three million: Montreal (3.82 million). That doesn?t necessarily mean MLB wouldn?t relocate to a market with less than 3 million. Milwaukee, MLB?s smallest market at 1.75 million, had the seventh highest attendance in 2011, drawing more than 3 million fans. Cincinnati drew more than 2 million and Pittsburgh came close to that mark. But unlike Tampa Bay, Cincinnati and Pittsburgh are two of the oldest franchises in MLB, which leads us to ...

2. Factor: Population Makeup.

This category requires four very key questions:

A) Are there a lot of transplants in the market, or are most of the residents natives?

One of the reasons the Rays fail to draw is because many of the residents of Tampa-St. Petersburg were born elsewhere. These fans go to Tropicana Field to see the Red Sox, Yankees or other teams because that?s where there true affiliation lies. So one key question to ask about relocation is how many of the residents are natives with no tie to an MLB team? That question also leads to ?

B) How far is the market from another MLB city and where do affiliates lie amongst the fans?

Two U.S. regions that have been cited in economic studies as ready to support an MLB team are Northern New Jersey and the Inland Empire section of Los Angeles, namely, Riverside-San Bernardino. There?s just one key problem these studies don?t take into account ? just how close these markets are to existing MLB teams. It?s extremely unrealistic to think sports fans in the suburbs of New York City are going to stop rooting for the Yankees and Mets, franchises they?ve grown up supporting for generations. Ditto for Southern Californians where fans have a long-time affinity to the Dodgers, Angels and Padres.

C) What kinds of people live here?

Every market is unique. In some markets, sports teams are an integral part of the fabric of the community (like in St. Louis and Milwaukee) while others get labeled as ?lousy sports towns? and rightfully so. For example, were it not for its sheer population size, it?s uncertain if MLB would even have a team in Miami, Atlanta or Toronto. Baseball will always come second in these cities, not first. Hockey rules in Canada and college football dominates the South. And even when these teams are good, sellouts are not a guarantee. Remember the Braves' run of 14 straight National League East crowns? There were Division Series games in which plenty of seats were empty. If MLB were to relocate, it would need to ensure the market has a strong culture of supporting its sports teams, but especially baseball, which leads us to ?

D) What?s the baseball history in the community like?

This is vitally important, especially for smaller markets seeking MLB?s attention. For example, does the market have a strong amateur program that has actually produced big leaguers? Has the market ranked among the top in Minor League Baseball attendance?

3. Final Key Factor: TV Landscape, Corporate Sponsorships and Regional Marketing

This is huge in baseball. With teams playing practically every day over a long 162-game season, reaching a large television audience is integral to a franchise?s success. The Brewers, for example, had the third highest television MLB viewership in 2011, according to Nielsen ratings, with only the Phillies and Cardinals with a larger per game TV audience. Although based in Milwaukee, the Brewers have statewide appeal across Wisconsin.

When the Rays eventually depart the Costco of MLB in 16 years, if not sooner, here are 10 markets in which the Rays might land, from most likely to least likely:

LABUAN: Despite being a duty-free island, Labuan has been listed as one of the key areas for consumerism programmes along with other states.

Labuan Domestic Trade, Cooperatives and Consumerism Office has conducted a study for the island?s potentiality to be part of the Domesitc Trade, Cooperatives and Consumerism Ministry?s long-term plan for consumerism development programmes.

Several shops on the island, including a strategically-located family restaurant called ?MoonStar, Restaurant, Grill and Barbecue? has been identified for the ministry?s transformation programme.

Its director Nursafariza Ihsan said three sundry shops in three villages ? Kampung Layang-Layangan, Kampung Pohon Batu and Rancha-Rancha ? would take part in the ?Tukar? programme.

The Tukar programme, created under the National Key Economic Area, is the the government?s initiative to transform sundry shops into modern outlets.

While MoonStar Restaurant, Grill and Barbecue will promote ?Menu Rakyat 1Malaysia (1Malaysia People?s Dishes), the ministry is promoting another programme to sell cheap food and drinks ? breakfast and lunch ? for as low as RM2.50 for breakfast and RM5 for lunch.

Nursafariza said efforts are in the pipeline to ease consumers? burden from the rising living cost.

?Our ministry has listed 13 eateries, with the latest being the MoonStar Restaurant, Grill and Barbecue to be promoted in the ministry?s portal.

?We?re targeting some 3,000 outlets taking part in the programme countrywide.

In Labuan, we only expected 10 outlets, but now we have more than that,? she said.

She said the Smart Consumers 1Malaysia Portal provides the participating Menu Rakyat 1Malaysia outlets.

?The programme will not only help consumers but also local retailers involved in the retail industry. The Tukar programme and Menu 1Malaysia promotion are the programmes that would greatly help to educate consumers and traders.

?They need to help each other without compromising the quality of products and offer reasonable prices,? she said.

Nur Azmin, manager of MoonStar Restaurant, Grill and Barbecue said the outlet would help the government promote the programme.

?We?ll keep the price low so long we can cover the cost.? he added.

To realise the Tukar programme objectives, Bank Rakyat will be offering RM20,000 to RM60,000 credit facility to sundry shop operators.

Labuan branch manager Abu Talib Harun said Bank Rakyat was committed to support the ministry?s mission to strengthen the co-operative sector as the nation?s economic growth thrust.

?We?re expanding our products and customer facilities to provide greater diversity to our customers. The Tukar programme will certainly help local retailers to move forward,? he said.

Under the Tukar programme, sundry shops will be spruced up with a new coat of paint, lay new tiles, re-arrange stocks and install computerised and management systems. ? Bernama

Iran would take pre-emptive action against its enemies if it felt its national interests were endangered, the deputy head of the Islamic Republic's armed forces was quoted by a semi-official news agency as saying Tuesday.

"Our strategy now is that if we feel our enemies want to endanger Iran's national interests, and want to decide to do that, we will act without waiting for their actions,'' Mohammad Hejazi told the Fars news agency, NBC News reported.

Iran announced air defense war games to practice protecting nuclear and other sensitive sites, the latest in a series of military maneuvers viewed as a message to the West that Iran is prepared both to defend itself against an armed strike and to retaliate.

The U.S. and Israel have not ruled out military action against Iran's nuclear program.

Visits to individual Iranian nuclear sites were also not part of the IAEA earlier visit three weeks ago.

But on Monday, Iranian state radio said the U.N. team had asked to visit the Parchin military complex outside Tehran that has been suspected as a secret weapons-making location and also to meet Iranian nuclear scientists involved in the country's controversial program.

"Iran's cooperation with the (IAEA) agency continues and is at its best level," added Mehmanparast.

Which anti-HIV drug combinations work best and why?

ScienceDaily (Feb. 19, 2012) ? Using a mathematical formula that carefully measures the degree to which HIV infection of immune system cells is stalled by antiretroviral therapy, AIDS experts at Johns Hopkins have calculated precisely how well dozens of such anti-HIV drugs work, alone or in any of 857 likely combinations, in suppressing the virus. Results of the team's latest research reveal how some combinations work better than others at impeding viral replication, and keeping the disease in check.

"Our study results should help researchers and clinicians develop simpler treatments, using either existing or new drugs, for people who are just starting therapy or people who have already tried and developed resistance to another combination," says senior study investigator and infectious disease specialist Robert Siliciano, M.D., Ph.D.

Siliciano, a professor at the Johns Hopkins University School of Medicine and a Howard Hughes Medical Institute investigator, and colleagues constructed the measurement tool, called the instantaneous inhibitory potential, or IIP, in the laboratory several years ago by analyzing the shape of drug dose-response curves in human immune system cells infected with HIV. They found that the curves' steepness reflects the extent to which small increases in the amount of drugs can further suppress attempts by the virus to bounce back, reproduce and spread.

Researchers say their latest study findings, to be published in the journal Nature Medicine online Feb. 19, along with other recent studies, provide valuable information to physicians about the potential strength of different combination drug therapies, and can help in streamlining and tailoring so-called highly active antiretroviral therapy, or HAART, to as few possible drugs as needed. Several hundred thousand of the more than 1 million Americans living with HIV disease are currently using HAART to fight the disease.

Among the latest study's key findings was that the most potent drug combos included the drugs efavirenz (a non-nucleoside reverse transcriptase inhibitor) and darunavir (a protease inhibitor.) According to the Hopkins team's calculations, the drug mix suppressed viral replication by more than a trillion times, enough to prevent infection of every single lymphocyte, or immune system cell, of which there are a trillion in the body.

The least-powerful drug was found to be one of the oldest anti-HIV medications, d4T, or stavudine (a nucleoside analogue reverse transcriptase inhibitor), which had the power to suppress viral replication by less than 10 times if used on its own (although, Siliciano points out, it works much better when taken in combination with other drugs.)

Siliciano says the most widely used combination, a single pill known as Atripla, consisting of tenofovir disoproxil fumarate (a nucleotide analogue reverse transcriptase inhibitor), emtricitabine (a nucleoside analogue reverse transcriptase inhibitor), and efavirenz, was able to reduce viral replication to as few as one in a billion.

Siliciano points out, however, that any drug combination which suppresses viral replication to the degree that out of every 100,000 lymphocytes exposed to the drugs, only one lymphocyte is likely to be infected (for five tenfold reductions) -- is sufficient to keep the disease in check, so long as people take their medication as prescribed.

"This means that overall access to anti-HIV medications could also improve as we develop simpler combinations of fewer drugs to achieve near total suppression," says Siliciano. Less than 7 million of the 34 million people worldwide infected with HIV are taking antiretroviral therapy, he notes.

The Johns Hopkins team based its new calculations on five years of analyzing just how antiretroviral drugs hinder key steps in HIV's life cycle, preventing it from replicating and infecting other immune system cells.

Scientists have for decades focused on multiple drugs targeting different enzymes that are key to the viral life cycle, thinking that multiple barriers along the chain could best halt replication.

Although the strategy worked, scientists had, until now, no theory to explain why some drug combinations worked well and others did not. Indeed, they point out, one of the newest classes of anti-HIV medications, so-called integrase inhibitors, did not work well as single drug treatments in laboratory experiments, but were highly effective in people when combined with other drugs.

Siliciano says that as a result of the Hopkins team's latest research and another of their recent findings, published in Science Translational Medicine in July, experts can finally demonstrate how different drug combinations disrupt and halt viral replication.

Researchers found that the steepest curves occurred when the drug targeted a stage in HIV's life cycle, in which many copies of viral enzymes, were needed. Citing protease inhibitors as an example, Siliciano says several copies of protease enzyme are needed to cleave the virus into hundreds of working parts before HIV can infect a new immune system cell. He goes on to say that "a level of inter-enzyme cooperation" is happening, specific to each stage of HIV replication.

"Our research shows that drugs like protease inhibitors really work like an on-off switch," says Siliciano. "Above a certain concentration, these drugs completely turn off viral replication. When you have only one copy of a viral enzyme needed in any key part of HIV's life cycle, a little more drug won't give you a lot more suppression; but, when you have more than one copy of enzyme needed for viral replication, then the dose-response curve for the drug will be a lot steeper, and a little more drug will completely shut off viral replication, which is what we want.

"It's gratifying to finally have a consistent metric for evaluating HAART medications that offers reliable information on how well they work in stopping HIV replication, and which also gives us a baseline target for suppression at less than one in 100,000 immune cells becoming infected in the presence of any drug combination," he adds.

The Johns Hopkins inhibition index was first developed to compare the level of viral inhibition from different drugs in different classes and to show how they could be graded.

Having measured the different potencies of many drugs, Siliciano conducted his next set of lab experiments to focus on the explanation behind different strengths of viral inhibition. The scientists measured the changes in the dose-response curves, plotting the results on graphs and comparing the sloping curves for each drug or combination of drugs.

Funding for this study, conducted solely at Johns Hopkins, was provided by the Howard Hughes Medical Institute and the National Institute of Allergy and Infectious Diseases, a member of the National Institutes of Health (NIH).

Besides Siliciano, other Hopkins researchers who took part in this study were lead investigator Benjamin Jilek, Ph.D.; Melissa Zarr, B.Sc.; Maame Sampah, B.Sc.; Alireza Rabi, B.Sc.; Cynthia Bullen, B.Sc.; Jun Lai, B.Sc.; and Lin Shen, M.D., Ph.D.

Currently, there are more than 34 million people in the world living with HIV, including an estimated 1,178,000 in the United States.

Truly revolutionary technologies rarely come along, but one is emerging right now.

Although the tech industry seems to be constantly changing, most new innovations are simply the next stage of improvement over something that already existed? combined with some new marketing buzzwords for everyone to get excited about.?For example, social media is nothing more than an extension of what the Internet has been providing since the mid-1990s. Tablet and mobile computing are nothing more than natural extensions of what desktop and laptop computing have been providing since the 1980s. And depending on how you define it, ?cloud computing? has technically been around since the 1960s and 1970s.
To be perfectly honest, there have only been a few truly revolutionary paradigm shifts in the history of computing technology. Everything else has simply been a refinement of the craft.

Mainframes
The first major revolution came in the 1960s and 1970s, with the adoption of mainframe computing. This new technology allowed organizations to offer much better service, at a lower price, on a much more massive scale? by automating processes which would?ve been expensive or impossible to perform by hand.
Mainframes completely revolutionized business and information management as we know it.
Of course, the problem with mainframes is that it placed the ?brains? of the organization in the hands of individuals who were technically knowledgeable? but lacked overall knowledge of what happens inside of the business and on the front-lines. But a new revolution would soon change all of that.
Desktops
When the first desktop systems came onto the market in the 1980s, the computing power which had once been the exclusive domain of mainframe operators was now placed in the hands of front-line workers and executives.
Now, business users could directly leverage the power of computers to automate their work and extend their capabilities as was never imagined possible before. This revolution led to work processing, spreadsheets, desktop publishing, digital video editing, and many other functions which went far beyond the simple reporting and database manipulation that took place in the mainframe server rooms.
Also, as business users became dependant on the power of these ?personal computing machines?, they began do demand portability and freedom to work from anywhere. This spawned the growth of laptops, tablet, and eventually mobile computing.Although smart phones have a form that?s completely unrecognizable from desktop computers, they perform many of the same functions in a more portable and convenient way.

Internet

Although networking and remote computing services have been around since the mainframe age, something very special happened in the 1990s.
Linux was invented in 1991, in an effort to make the power of Unix available freely to smaller start-ups, programmers, students and the general population. Roughly about the same time, Tim Berners-Lee created the early foundation for the World Wide Web.
This made worldwide networking more accessible to the general population, and forced telecommunications companies to upgrade their infrastructure to keep up with demand from companies and individuals for this new communication channel.
We?ve come a long way since then. And today, most business is conducted over the Internet? and at a massive scale.
Even ?new? technologies like ?Social Media? are simply extensions of what the Internet was originally intended to do. (It?s also important to note that certain technologies, such as email and remotely-hosted applications have actually been around since the mainframe era)

Big Data
You may not realize it yet, but a massive paradigm shift has been occurring over the past few years? and it will completely change the way you use technology forever. This new innovation will be just as life-changing as mainframes, personal computers and Internet technology. But the change is taking place so subtly that you won?t even realize it?s happened until it has already become an essential part of your everyday life.
Lower storage costs have enabled organizations to produce, collect and retain data on a scale that has never been seen before. In fact, this exponential data growth has reached a point where traditional relational database models are no longer sufficient for working with this information.
A perfect example would be Google, which receives billions of search queries every day? and must search through petabytes of indexed content within milliseconds to return the most relevant results. Data analysis on this scale would simply not be possible with structured relational databases. Instead, a completely new way of working with data was required.?Part of the problem had to do with the fact that relational databases were primarily designed for high-volume, high-accuracy transactional data processing? similar to what a bank would require. But these data models were poorly designed for analyzing and extracting information from massive data pools.
New ?non-relational? database technologies like Hadoop, Cassandra, NoSQL, and MapReduce have unlocked the potential of this stored information by making it easy, cheap and practical to extract value from stored database data and even unstructured data.

Business, finance, government, commerce, medicine and a number of other fields will reap enormous benefits from the powerful capabilities of Big Data.
For years, scientific research has been limited by technology?s ability to store, analyze and simulate massively complex models. Big Data will permit scientists and researchers to make breakthroughs in the areas of weather prediction, genetic medicine, particle physics, and astronomy. And governments will leverage the statistical analysis capabilities of Big Data to promote greater transparency, and to establish more successful economic policies.
Big Data is a big deal. And its effects on society will be on par with those generated through Mainframes, Personal Computers, and the Internet.
Big changes rarely come along. So it?s really exciting to see ourselves at the beginning of this incredible new era in the history of computing.

Before starting ADA accommodations process, ask basic question: Is ...

Finding a reasonable accommodation is a two-way street. Both the employer and the disabled employee are supposed to engage in the ADA?s interactive accommodations process. The idea: Working together, the two can reach a workable accommodation that best suits both business needs and the employee?s particular disability.

But part of that interactive process includes determining whether the employee is, in fact, disabled. If he?s not, the process need go no further.

That?s why the process should start with gathering facts from the em??ployee and his doctors so you can assess the employee?s condition and determine if it impairs a major life function such as walking, talking, seeing, breathing and caring for oneself.

The report may surprise you. While an employee may think of his condition as disabling, medical specialists may not. If the doctors don?t think the employee?s condition impairs a major life function, you needn?t go further. The employee isn?t covered by the ADA because he?s not disabled.

Recent case: Rommel Griffin worked for UPS for 28 years, mostly as a supervisor on the 2 to 10 p.m. shift. Griffin has diabetes, which requires him to eat carefully while managing his insulin levels.

Griffin took some time off for stress-related ailments. When he returned, he was informed that his shift would change to the overnight shift. He pro??tested, claiming that working overnight would aggravate his diabetes. He wanted to switch to the day shift.

UPS immediately began the reasonable accommodations process by re??questing information from Griffin?s doctors. At that point, all UPS had was a note from his behavioral health doctor suggesting that Griffin ease back into full-time work by just working part time for the first three weeks.

Griffin?s doctor did complete a medical form suggesting he could best manage his illness on the day shift. But the doctor had checked ?No? on a question about whether Griffin?s condition substantially limited a major life activity other than working.

HR then informed Griffin that, based on his own doctor?s medical report, he was not disabled and thus not entitled to reasonable accommodations. He immediately submitted his retirement notice.

Then Griffin filed a failure-to-accommodate lawsuit.

UPS argued that Griffin had abandoned the reasonable accommodations process. He could have provided more information on how his condition substantially limited major life functions, but he chose instead to retire.

The court agreed that UPS had met its obligations and dismissed Grif??fin?s case. (Griffin v. United Par??cel Serv??ice, No. 10-30854, 5th Cir., 2011)

Remember: Medical conditions can change over time. Just because an employee?s condition wasn?t initially severe enough to substantially impair a major life function doesn?t mean it might not become disabling later. With????????in reasonable limits, handle subsequent accommodation requests by asking for updated medical information.

Also remember that the obligation to accommodate is continual. If needs change, so may the accommodation.
